Human Subjects Project Application Forms
A completed Application to Collect Human Subjects Data must be reviewed and approved by an appropriate reviewer before an investigator can begin contacting subjects or collecting data. See Who Needs to Review My Project? to determine the appropriate reviewer for a project. If a project requires review by one or more members of the IRB, the completed application form below, and all attachments, should be emailed to irb-administrator@stolaf.edu for assignment to an IRB reviewer.

(Student applicants should first submit this form to the instructor/project supervisor, who will submit the form to the IRB administrator, indicating his/her own approval of the application.)
Authorization to Contact Prospective Participants
Application for Continuation of IRB Approval
(This form is for projects previously approved by expedited or full board review, requesting continuation of subject participation beyond the original approval period.)
Modification of Previously-Approved IRB Application
(This form is used to seek IRB review of proposed changes to a human subjects project that has a current IRB approval.)
