INFORMATION FOR FIRST-YEAR STUDENT ATHLETES

Welcome to St. Olaf College for the start of a new and exciting phase in your life. This memo is intended to guide you through the process of obtaining medical clearance to begin practicing as an intercollegiate student-athlete (this does NOT pertain to club, intramural, recreation, and dance athletes). Use this page as a checklist.
1. Make an appointment with your family physician for a physical exam. This date must be with 6 months of your first practice.
2. Go to http://www.stolaf.edu/athletics/sportsmed/
- Read the two documents Insurance Information and Medical Care Information. Ask your parents to read these documents.
- Verify that your insurance policy covers athletic injuries and to what limits. Consider listing the Allina Medical Clinic of Northfield as your primary clinic and Dr. Scott Koehler as your primary physician with your insurance company. - Click on Insurance Form (under First-Year Student-Athletes) and log-in: http://www.stolaf.edu/admitted
- Enter the insurance information and your emergency contact information and submit.
- Download and print the two forms: Health History and Physical Exam forms.
- Complete the Health History Form using BLACK ink only. Provide explanations for any “YES” response. Sign and date the form.
3. Pre-Participation Physical Exam (PPE):
- Take the completed Health History form and the REQUIRED Physical Exam form to your physician for a physical exam.
- It is highly recommended that all endurance athletes (cross-country runners, Nordic skiers, distance swimmers, and distance track runners) and vegetarian athletes be tested for Ferritin levels, with results indicated on the PPE form. Click on Ferritin Information for more information.
- Before leaving the doctor’s office, make sure that the doctor has initialed the Health History form, has indicated that you are cleared for physical activity, and has signed and dated the Physical Exam form.
4. Return forms to the Athletic Department (using Athletics Privacy Envelope)
- Send two copies each of the Health History and Physical Exam forms with two LEGIBLE copies of the front and back of your insurance
5. Deadlines
- You will NOT be allowed to take part in any physical activity with the team prior to achieving medical clearances. The process may take 7-10 days once we receive all the forms, so please stick to the deadlines.
- Fall Athletes: July 15
- Winter & Spring Athletes: August 15

- Make copies of all forms for yourself.

- Fax: 507-786-3721 is a secure fax line in the Athletic Training Room. Faxed copies of the forms will be accepted for temporary purposes only. Hardcopies of the original forms are required.
