There are three parts to the Proposal Clearance form. You will need to complete the:
COI-Financial_Interest_Disclosure_Form 5-12-08
Conflict of Interest Form(COI): Provides guidance for what is considered a conflict of financial interest for all employees. Identifies the procedures to follow if a conflict of interest arises. Provides special guidance for grant funded and similar academic projects.
