Information for St Olaf students already covered under the plan:

Claim Procedures in the event of an injury or illness:

  1. Print an identification card if you have not received one from the health insurance company.
  2. Report to the Health Services so that proper treatment can be prescribed.
  3. If away from campus, or if Health Services is closed, consult a physician.
  4. ACI must be notified within 30 days after the date of the injury or illness.
  5. A claim form MUST be completed in full and signed.
  6. Send the claim form within 90 days from the date of injury or illness.

Claim procedures to receive reimbursement for prescriptions:

  1. You must pay for your prescription at the time of purchase.
  2. A claim form MUST be completed in full and signed.
  3. Send the form along with the receipt within 90 days.
  4. Retain a copy of all information for your records.
  5. Reimbursement after $8.00 copay is 80% of the reasonable expense.
