Template
The template below provides the exact form that you must follow for
the bibliographic information. Please transfer this form to your
own disk and use it as a template for each review.
*Note that you will
replace the phrases that are after the* (including the *'s).
You can transfer this form to your disk
with a cut and paste technique or by using the "Save As"
command on the Netscape File menu. (Save in Source format.)
Then just erase all of the lines before (and including)
the lines that say "Start at the line below" or "Stop
at the line above"
