Destination St. Olaf is designed to contain all information necessary for your successful matriculation to St. Olaf College. A few helpful hints:
#1 All the gold-colored pages need to be returned to St. Olaf College in one of the reply envelopes found in the back by July 15th, 2009.
#2 All the green-colored pages need to be returned to St. Olaf College in one of the reply envelopes found in the back by July 15th, 2009.
#3 All the tan-colored pages need to be returned to St. Olaf College in one of the reply envelopes found in the back by August 1st, 2009.
#4 Upon receipt of these forms, the Dean of Students Office will ensure they are sent to the appropriate offices on campus. Online forms get directed to the appropriate offices automatically.

The following are gold-colored and green-colored forms that must be returned by July 15th:

- St Olaf Procedure to Release Information (gold-color, Student Life Section, page 21)
- St. Olaf Health Services Report of Medical History & Immunization Record Form (Student Life Section). Form is printed on green-colored form and needs to be returned by July 15th.
- St. Olaf Sports Medicine Health History & Pre-Participation Physical Exam (PPE) Form (Student Life Section). Form is printed on green-colored form that must be returned by July 15th for Fall Athletes. (Student Life Section).
The following are tan-colored form that must be returned by August 1st. If you have a completed paper copy of Medical History, Immunization or First Year Athlete form, please send it back in the appropriate envelope in the back of the binder.
